To all net control operators:
If the current repeater becomes inoperable or unreliable, net control should move the net to the next frequency on the list.
One person who is capable of monitoring both frequencies should remain on the original frequency to direct any newcomers to the active frequency.
If possible, Skywarn nets will run on the 147.015 repeater.
If necessary to move, the order will be
1) 147.165 repeater
2) 442.9 repeater
3) 145.130 repeater w/ 88.5 Hz tone
4) 147.015 simplex

Instead of generating a new form, I found the ICS 211A form that should do the job quite well. Just disregard the block on reporting agency.

Activating a Hendricks County Skywarn Net
1. A Hendricks County Skywarn Net will be activated if _any_ of the
following conditions exist:
a. A Severe Thunderstorm Warning or Tornado Warning is issued for
Hendricks County.
b. A Severe Thunderstorm Warning or Tornado Warning is issued for
Boone, Montgomery, Putnam, Owen, Morgan, or Marion Counties and the
severe weather appears to be tracking towards Hendricks County.
c. The National Weather Service, either directly or through the
Central Indiana Skywarn Net, asks Hendricks County to activate a net.
